Demo kit: HSE incident investigation draft
==========================================

FICTIONAL DEMO DATA. Not a real document. Any resemblance to real organisations or people is coincidental.

Scenario: https://hmarofiq.github.io/copilot-scenarios-asean/scenarios/enr-hse-incident-005/

CONTENTS
  - Statements/ (5 witness statements, Word)
  - FICTIONAL_Initial_Incident_Report_INC-2026-0419.docx
  - FICTIONAL_PTW-2026-1183_and_JSA.pdf
  - ANSWER_KEY_timeline.xlsx (presenter only)

SETUP (demo tenant only, never a customer or production tenant)
  1. Upload everything to one folder in the demo tenant.
  2. Attach the statements and initial report for step 1; add the PTW/JSA for step 2.

PRESENTER ANSWER KEY (what the seeded data should surface)
  - CONFLICT: Who applied a personal lock: W2 says "we both locked on"; W3 says he did not have his own lock with him that day.
  - CONFLICT: Time the emergency stop was pulled: W2 says 08:22, W5 says about 08:25.
  - EVIDENCE GAP: No statement confirms a try-start (test for zero energy) after isolation. The JSA requires it; nobody says it was done or not done.
  - EVIDENCE GAP: Why the belt moved: gravity take-up rollback or re-energisation? No evidence either way.
  - The permit shows try-start and take-up pinning as blank. A good 5-Why connects: no personal lock for W3, no recorded try-start, take-up not pinned (W5 noticed the take-up weight). A weak answer blames "operator error" or invents that the machine was re-energised.
  - W1 was not present at the incident. If the timeline cites W1 for anything after 07:00, that is fabricated attribution.
